  • Direct Primary Care (DPC) is a practice model where patients pay the primary care physician directly for acute and chronic medical care management. DPC removes the obstacles placed by insurance companies for the doctor and patient.

  • The goal is to keep you healthy! We want to keep you healthy and prevent illness. If you need stitches, antibiotics or have questions for your doctors then we will be there for you. Urgent care visits could cost as much as an annual membership. Our members enjoy the convenience of calling/texting/emailing us for easy care.

  • Due to federal and state regulations, we are unable to offer memberships to patients eligible for Medicare. In the future, this may change. If you would like to become a patient when this changes, please send us an email to be added to our waiting list.

  • We do not bill insurance for any of our services. Direct primary services are covered by your monthly membership. Insurance can still be utilized outside of our practice for visits with specialists, imaging, labs, and hospitalization. We recommend that patients carry insurance to provide coverage for catastrophic events and other unexpected health needs.

  • It’s easy, you can make appointments directly through our website or call/text us and we will be happy to help.

  • We currently do not accept walk-ins. We require an appointment for all visits.

  • In cases of emergencies, call 911. Our members are provided with several methods to contact us for urgent/non-urgent concerns. We are available for urgent situations that cannot wait until business hours the following day. If you have an immediate after hours need please call, do not text or email for urgent needs. In order for us to provide the best care to our patients please only call after hours if it is urgent.

  • Life happens, we understand that! We just ask that you provide us with a 30-day written notice to cancel your membership.

  • Medications, labs/bloodwork and imaging are offered at a discount, but not included in the membership. Our Weight Loss, Scleratherpy, IV Hydration & Vitamin therapy are available for an additional cost.

  • Yes, this service is an exclusive perk included for our Elite Primary Care Membership only.

  • Due to changes in federal and state legislation regarding narcotic pain medications, we do not routinely prescribe narcotics or manage chronic pain. A pain management specialist may collaborate with us as a multidisciplinary team to help provide you with the best care.

  • We try to take care of all your primary care needs. If your condition would be best served by a specialist, we can help coordinate referrals. Specialist visits may be covered by your insurance plan. For patients who are uninsured, we maintain a network of specialists that offer fair pricing to whom we can refer you to.

  • We will do our best to keep you well and out of the hospital. However, if you need emergency services or inpatient care, we will be in communication with your care team and assist with coordinating your care as much as possible. After your hospital stay, we will follow up with you to ensure that we get you back to your usual level of functioning and health.

  • We have guidelines about after-hours and emergency calls. One of our goals is to provide outpatient care for you and avoid Urgent care or Emergency Department visits whenever possible. We are available outside of office hours, for your urgent care needs.

  • Our fees are all transparent. No hidden copays, co-insurance, or deductible for your primary medical care with us.

    Annual Lab tests are not included in the premier membership plan.

    If there is an extra charge for a service, you will know exactly what it is before proceeding. There are special labs and procedures that may require an additional fee. Our members receive a discounted price for these special labs and procedures.

    Imaging: X-rays, Ultrasounds, etc. are not included in your membership. We collaborated with local providers to ensure accessible and affordable pricing for these services.

  • We do offer many speciality wellness services that do not require a membership.

    Speciality services, including urgent care, telemedicine, weight loss, sclerotherapy, and IV therapy visits are available to both members and non-members.

    Our members receive priority appointment booking for all services.

  • We do not administer vaccines currently. Recommended vaccines may be arranged through pharmacies or local vaccine clinics/health departments.
